Utah, Starvation Campground, Starvation State Park.  

Directions:

On Main Street in downtown Duchesne go West.  At the West end of town while on W. Main St, take a right onto State Route 311.  Follow State Route 311 to the campground. 

The campground does not appear to have an address however, the Campbase map is accurate for the location of the campground.

Starvation Campground in the Starvation State Park has cabins, tenting, and RV camp spots.  Full hookups are available at a limited number of sites.  Partial hookups are also available.  The campground is open all year.
